ในโอกาสนำเสนอผลงานวิชาการรูปแบบ Oral Presentation ในการประชุม ISHAM Asia 2026 ระหว่างวันที่ 10–12 กันยายน 2569 ณ กรุงกัวลาลัมเปอร์ ประเทศมาเลเซีย 🇲🇾

ในหัวข้อเรื่อง “Disseminated cryptococcosis mimicking tuberculosis: A case report of pleural and lymph node involvement in an advanced HIV patient”

รีวิวประเด็นสำคัญจาก CLSI M100 ฉบับที่ 36 (2026) มีการปรับปรุงสำคัญ 11 ประเด็น โดยสรุปเป็น 3 แกนหลักครับ 📚

1️⃣ Expanding AST Options: เพิ่มทางเลือกในการตรวจและการรักษา

• เพิ่ม breakpoint ของ aztreonam-avibactam สำหรับ Enterobacterales โดยเฉพาะเชื้อกลุ่ม MBL-producing CRE
• ปรับบทบาทของ sulbactam-durlobactam ใน Acinetobacter spp. เป็น cascade reporting หลัง imipenem หรือ meropenem เพื่อสงวนยาไว้สำหรับ CRAB ที่เหมาะสม
• เพิ่ม MIC breakpoint ของ TMP-SMX สำหรับ β-hemolytic streptococci (Tier 4) โดยหากทำการทดสอบควรใช้ broth microdilution ร่วมกับ lysed horse blood เนื่องจากมี thymidine content ที่น้อยกว่า sheep blood

2️⃣ Refining AST Criteria: ปรับเกณฑ์ให้แม่นยำมากขึ้น

• ลด breakpoint ของ amikacin, gentamicin และ tobramycin ใน Acinetobacter spp. ลง 1 two-fold dilution ทำให้ผลเดิมบางส่วนอาจเปลี่ยนจาก S เป็น I
• นำ ECVs ของ Burkholderia cepacia complex ออกทั้งหมด เนื่องจากไม่มี CLSI breakpoint หรือ ECV สำหรับใช้ตัดสินทางคลินิก
• ปรับ cefiderocol disk diffusion breakpoint ใน S. maltophilia จาก S ≥15 เป็น S ≥17 mm ขณะที่ MIC breakpoint ไม่เปลี่ยนแปลง
• ปรับเกณฑ์ MIC ของ ceftriaxone และ cefixime ใน N. gonorrhoeae ให้เข้มงวดขึ้น เพิ่ม I/R และถอดเกณฑ์ disk diffusion ออกชั่วคราว

3️⃣ Advancing Laboratory Practice: ได้ผลตรวจเร็วและนำไปใช้ได้จริงมากขึ้น

• แยกตารางของ Salmonella spp. และ Shigella spp. ออกจาก Enterobacterales อื่นเพื่อให้ใช้เกณฑ์เฉพาะเชื้อได้ถูกต้อง
• เพิ่มเกณฑ์ direct AST ของ piperacillin-tazobactam จาก positive blood culture ใน Enterobacterales โดยอ่านผลที่ 16–18 ชั่วโมง และรายงาน R เมื่อ zone ≤17 mm
• ไม่ควรแปลผลหรือรายงาน tobramycin จาก direct AST ใน Acinetobacter spp. จนกว่าจะมีเกณฑ์ใหม่
• ปรับปรุง anaerobe cumulative antibiogram ใน Appendix D โดยรวบรวมข้อมูล 48,894 isolates ระหว่างปี 2017–2024 เพิ่มคำแนะนำการทดสอบความไวต่อยา amoxicillin-clavulanate และ ceftriaxone เพื่อช่วยสนับสนุนการเลือกยารักษาเบื้องต้น

เพราะผล AST ที่ถูกต้อง ไม่ได้ช่วยเพียงให้รายงานผลได้แม่นยำขึ้น แต่ยังช่วยให้แพทย์เลือกการรักษาได้เหมาะสมและปลอดภัยยิ่งขึ้นครับ

Associate Professor Pornpan Koomanachai, MD, Assistant Dean of International Relations at Faculty of Medicine Siriraj Hospital, recently returned from her fifth visit to UCLA 🇺🇸, continuing to expand medical education collaboration between these leading institutions.

🤝 During this visit, Assoc. Prof. Koomanachai met with UCLA's Global Health Program team to discuss student feedback methods and connected with Dr. Sadath Sayeed, whose energetic problem-solving approach left a strong impression.
