🩺 Would you take a genetic test that could change your heart disease risk forever? A 14-year study reveals that 24% of patients with inherited heart conditions have a disease-causing variant, yet only 8% see changes in treatment. Are we underutilizing genetic testing? BIOADAPTOR RCT: This is the first report of 2-year outcomes comparing the bioadaptor implant with a contemporary drug-eluting stent. At 2-year follow-up, fewer target lesion failure events were observed in patients treated with the bioadaptor. https://bit.ly/3DsRcQ0

LBCT: Results of the CRABBIS RCT show that during stepwise provisional stenting in left main or large bifurcations, the proximal optimization technique (POT)-Kissing-POT ballooning sequence is associated with better stent configuration compared to the POT-Side-POT.

Higher density, lower risk coronary artery disease plaque? Higher coronary artery calcium density confers a lower risk of future cardiovascular disease events with a ~20% lower risk for each standard deviation increase in density in this systemic review and meta-analysis.

Emotional Stress Triggered Severe Mitral Regurgitation: The Multifactorial Interplay of the Mitral Valve Mitral regurgitation (MR) is a dynamic disease and is highly influenced by hemodynamic loading conditions and left ventricular (LV) geometry.1,2 Rare entities of severe MR without evidence of mitral valve (MV) structural abnormalities and LV dysfunction have been reported.3,4 The etiology and the pa...

This analysis of 3,363 patients with aortic stenosis (AS), those with low-gradient AS and reduced left ventricular ejection fraction, and those with paradoxical low flow, low gradient AS had significantly higher rates of death or heart failure hospitalization. In patients with heart failure with reduced ejection fraction on guideline-directed medical therapy, transcatheter aortic valve replacement (TAVR) did not affect the primary hierarchical composite endpoint compared to clinical aortic stenosis surveillance. Preemptive TAVR for moderate aortic stenosis was safe and may provide early and clinically meaningful quality-of-life benefits.

The restitution threshold index (RTI) in persistent atrial fibrillation was associated with a reduced left ventricular ejection fraction (LVEF), whereas the mean heart rate was not. The RTI could be used to predict LVEF improvement after catheter ablation.

In ♂️ and♀️ the most prominent risk factors for new-onset heart failure (HF) were hypertension and obesity, but with ⬆️ risk of new-onset HF in women. New-onset HF in both ♂️ and ♀️ was associated with pathophysiological pathways related to neutrophil degranulation and immunomodulation.

clinical case describes a patient with anterior STEMI owing to subocclusion of the left anterior descending artery, presenting with STE in leads DI, aVL, and V1 to V6, and ST-segment depression in aVR. The significance of this case lies in the overlooked role of STD in aVR in diagnosing anterior STEMI! https://bit.ly/41ls71l

Aortic Stenosis With Dynamic Left Ventricular Outflow Obstruction: Diagnostic and Management Challenges—A Case Series Although rare, aortic valve stenosis (AS) can coexist with hypertrophic cardiomyopathy (HCM) with obstruction. In an estimate from 207,880 patients from the National Inpatient Sample who underwent transcatheter aortic valve (AV) implantation from 2014 to 2018, about 0.38% had concomitant HCM.1 When....

Association of Cardiac Sarcoidosis and Ulcerative Colitis: A Case Report and a Brief Review of the Literature Sarcoidosis is a systemic disease characterized by a chronic immune-mediated inflammatory status leading to non-necrotizing granulomas formation, which may theoretically affect any organ, including the heart. Importantly, cardiac involvement is one of the main prognostic determinants in these patien...

This analysis of 42,887 patients from the SWEDEHEART registry shows comparable rates of MACE (31.3% vs. 32.2%) between instantaneous wave-free ratio vs fractional flow reserve-guided percutaneous coronary intervention and no difference in all-cause mortality over 5 years. https://bit.ly/43fJ1RA

Prognostic Value of Echocardiographic Coupling Metrics in Systemic Sclerosis–Associated Pulmonary Vascular Disease Ineffective right ventricular (RV) adaptation to increasing pulmonary arterial (PA) afterload in pulmonary vascular disease (PVD) significantly contributes to morbidity and mortality. Pulmonary vascular disease in systemic sclerosis (SSc) arises through various mechanisms, yet detecting abnormal con...
