Sports Physical & Immunization Clinic for
LPS High School and Middle School Athletes

LMH Health will be at LHS on Wednesday, May 29th to give sports physicals for
interested LPS High School and Middle School athletes. The cost is $25 (cash or check).
No Appointments. Doors will open at 6:00 and close promptly at 7:00 pm. Half of the cost
will be donated to the district sports medicine budget.
Also at the event, Lawrence Douglas County Public Health will offer required immunization for
students 7th grade - 11th grade including Tdap, Meningococcal, and Hepatitis A vaccines.
