How to Apply
You must be a high school junior or senior with an un-weighted GPA of 2.5 or higher to qualify for CHS’s Allied Health and Digital Multimedia or a 3.0 un-weighted GPA for the AA (Associate of Arts) degree program. You must have a minimum of 11 high school credits at the end of your tenth grade year and be on track for earning a standard Florida high school diploma.
Students may apply in the spring of their Sophomore or Junior year of high school. Public information meetings will be held in the month of January and February each year on the campus of Polk State College to provide detailed information about the school and the application process. All potential applicants are strongly encouraged to attend one of these information sessions.
Information Session Dates for 2017 are on Tuesday, January 31, 2017, Wednesday, February 8th, and Thursday, February 16th, 2017 at 6:30 PM in the LTB Auditorium in the Lakeland Polk State Campus.
All applicants are required to take the PERT Test.
The PERT Test is an un-timed assessment taken on a computer.
The test may be taken Monday – Saturday during the posted testing hours at the TLCC on any campus. To take the placement test, a placement testing “admission ticket” is required. Applicants must fill out an online admission application if testing at a Polk State campus at this link: https://passport.polk.edu/Genesis/student/applogon.jsp. The admission ticket must be printed out after completion of the online application and brought to the campus for testing. Applicants must know their Social Security numbers and have photo IDs. Applicants must bring photo ID to the test location. All testing supplies are furnished. Calculators are NOT permitted. The lottery application deadline is Friday, March 3, 2017 at 4:00 PM.
