A "perfect" note isn't always the best note.

As documentation requirements have evolved, many notes have become longer, more templated, and increasingly difficult to interpret. When every encounter looks the same, it becomes harder to identify what was truly relevant to the patient's care.

The goal of documentation has never been to create longer notes. It's to create clear, accurate documentation that tells the patient's story and supports the work performed.

As auditors and educators, we should encourage documentation that reflects clinical judgment, not just checklist completion.
