We were proud to have Debbie Packard represent Acti-Kare at Valley Medical Center’s Case Management Resource Fair in Renton!

Deb spent the day connecting with case managers, social workers, discharge planners, and other community providers who help patients and families prepare for a safe transition home.

These relationships matter. When someone is leaving the hospital, families often need practical support with personal care, meals, mobility, transportation, medication reminders, and safety at home.

Scheduling, recruiting, client care, community outreach, billing, and human resources - everyone here plays an important role.

Most families caring for someone with dementia have never heard about the Medicare program may cover 72 hours of in-home care a year, at no cost.

It's called the GUIDE program, and the qualifications are simpler than people expect — a moderate to severe dementia diagnosis and Traditional Medicare (not Medicare Advantage).

This short video breaks down how it works in about a minute. If you're caring for a parent or spouse with dementia in King, Snohomish, or Pierce County, this one's worth a watch.

Most families caring for a loved one with dementia have never heard of Medicare's GUIDE program.

It's a benefit that launched in July 2024 — and for families in King, Snohomish, and Pierce Counties who qualify, it can mean up to 72 hours of no-cost in-home respite care each year.

We are set up to provide the in-home care for GUIDE-enrolled families in our area, and we put together a full resource page and a downloadable family guide to help families understand what it is, who qualifies, and how to get started.

If your loved one has a dementia diagnosis and is on Traditional Medicare (not Medicare Advantage), this is worth reading. The link is in the comments.

Feeling overwhelmed as a family caregiver?

That constant worry, the sleepless nights, the feeling that it is 100% on you all the time... those are signs that you may need support.

And asking for help does not mean you are giving up on your parent.

It means you are being practical, thoughtful, and honest about what your family needs.
