The patterns that kept you safe in residency-perfectionism, responsibility, people-pleasing-are still running the show. They built your reputation at work. They're also why you snap at your kid over spilled milk, why you're charting at 10 PM, why 'present at home' feels impossible on high-stakes days.

Here's the thing: you don't have to choose between being excellent and being present. You just have to recognize the pattern and learn to shift it.

That's mental fitness. And it's learnable.

Here's what most healthcare parents miss: your steadiness is contagious. When you walk through the door calm, grounded, and actually *here*-not spinning on your last chart or replaying a difficult conversation-your kids feel it. They settle. They trust the evening.

But presence isn't a personality trait you either have or don't. It's a skill you build. And like any skill, something gets in the way.

Maybe it's the Hyper-Achiever in you, still optimizing when you should be present. Maybe it's the Pleaser, managing everyone else's comfort before your own. The Controller, needing to orchestrate every detail. The Stickler, stuck on what went wrong.

These are your Saboteurs-and they're not your fault. They kept you safe and productive in medicine. But at home, they cost you presence.

The good news? You can quiet them. In 90-second blocks. Between patients, on the car ride home, before bed.

You snapped at your kid over nothing. Your partner asked a simple question and you felt controlled. That's not a character flaw-that's your Saboteur on a high-stakes day.

Here are the three patterns I see most in healthcare parents under pressure:

Hyper-Achiever (Perfectionist Spiral). Post-call, you notice your kid's room is messy and suddenly you're deep-cleaning at 10 PM instead of being present. The nervous system is still in "optimize and execute" mode. The 90-second shift: name it, take three slow breaths, and ask yourself: "What matters most in this moment?" Often it's connection, not perfection.

Pleaser (Guilt Spiral). You missed bedtime again. Now you're overcompensating-saying yes to everything, exhausting yourself further. The Saboteur tells you that you're failing as a parent. The 90-second shift: pause, place a hand on your heart, and remind yourself: "I showed up at work with skill and care. My child needs me steady, not guilty."

Controller (Reactivity Under Fatigue). Partner asks, "How was your shift?" and you snap. You feel like your autonomy is being questioned when really you're just depleted. The 90-second shift: notice the tightness, breathe into your shoulders, and choose one sentence to share instead of reacting.

Which one shows up most for you? Drop it in the comments-I'd love to know what your trigger moment looks like.

You show up steady and decisive in the OR, at rounds, at the bedside. Then you walk through your front door and feel the weight of everything you've had to compartmentalize-the parts of you that want to be fully present, unhurried, *there* for your kids and partner.

That anxiety you feel between those two worlds? It's not a malfunction. It's your nervous system flagging a mismatch.

When your clinical identity (composed, in control, problem-solving) and your home identity (present, connected, available) don't align-when one demands everything and the other gets leftovers-anxiety shows up as a messenger, not a diagnosis.

The question isn't *How do I fix this?* It's *What is this actually telling me?*

Often it points to a boundary that needs clarity. A value that's been sidelined. A part of you that's been performing so long you forgot it was an act.

Your personalized Saboteur Assessment reveals which patterns-perfectionism, people-pleasing, control, rigidity-are driving the split. From there, you can decode what your anxiety is really signaling and align your life around it.

Ready to listen instead of medicate? Request your Saboteur Assessment invitation below-it's the first step to understanding what your nervous system is trying to tell you.

That tight chest between patients. The 2 a.m. inbox spiral. The guilt when you're physically home but mentally still charting.

You're not broken. Your anxiety isn't a symptom to suppress-it's your nervous system sending you data.

Often, that data is simple: there's a gap between who you're performing as (the clinician, the overachiever, the one who handles it all) and who you actually want to be (present, boundaried, steady). Your body knows the difference. It's protecting you.

The question isn't "How do I make this feeling go away?" It's "What is this feeling trying to show me about how I'm living?"

That's where the real shift happens.

We use something called the Saboteur Assessment to decode which pattern is driving your anxiety-whether it's the Hyper-Achiever pushing you to do more, the Pleaser saying yes when you mean no, the Controller trying to manage everything, or the Stickler holding impossible standards.

Once you know your pattern, you can actually change it. Not with willpower. With mental fitness.

Ready to decode your signal? Request your personalized Saboteur Assessment below-it takes 5 minutes and shows you exactly which Saboteur is running the show.
