The first of a two-part discussion on Optimized Bone Grafting, a paper I published in Periodontology 2000 that became the most cited article in the journal in 2024.

The goal of this work was simple but long overdue. To bring clarity to bone graft selection by grounding it in biology rather than preference or habit. Autogenous bone, allografts, xenografts, and synthetics each behave differently, and understanding when and why to use them is critical for predictable outcomes.

In this first part, I walk through the foundational concepts behind graft materials and the biological properties that define them. The second part will build on this framework and move deeper into clinical application.
