Just a reminder that any student who plans to participate in athletics must have a completed physical uploaded to Final Forms before they are permitted to practice.

Please see the information below regarding an opportunity to receive a free sports physical.

It's that time of year again and we are offering FREE school sports physicals! We have 2 dates available, Monday July 27th and Wednesday July 29th from 1pm-4pm on both dates.
The IHSSA Sports Physical form needs the parent/guardian portion filled out BEFORE the appointment. You can download the form at https://www.ihsaa.org/resources/health-well-being scroll down to the Pre-Participation Physical Evaluation and print it from there.
