. Dr. Kawan A. Swain is a Nephrologist who practices General and Interventional Nephrology. He currently serves as medical director of a dialysis unit overseeing home hemodialysis and peritoneal dialysis. Dr. Swain is Assistant Professor of Medicine in the Division of Nephrology at UNC School of Medicine in Chapel Hill NC. Dr. Swain is board certified(ABIM) in Internal Medicine and Nephrology. He received his medical degree from Brody School of Medicine in 2010, completed his residency in Internal Medicine at Wake Forest Baptist in 2013, and completed a fellowship in Nephrology at UNC-Kidney Center in 2015.

BCES would like to highlight minorities in medicine with the introduction of our program. This program is more than just a hashtag. The mission is to follow minority youth from high school to medical school and provide them with the necessary resources to become successful physicians who will one day help service their own communities. We’d like to use this hashtag to encourage more minority youth to consider careers in medicine. Our hope is for mass exposure showcasing minority physicians throughout the world. Representation is what we need to make OUR America and OUR communities great again.

A diseased heart removed prior to a transplant and keeping itself active even outside the body, beating peacefully on its own. Fascinating!!

The conducting system provides the heart its automatic rhythmic beat.
This video demonstrates a classic presentation of cardiac automaticity, which is the property of a specialized cardiac cells called pacemakers to generate spontaneous electrical impulse.

Cells normally capable of generating an impulse and maintaining a heart rate commensurate with the body’s need are the pacemaker cells in the sinus node and AV node among others, which fire at regular intervals and leads to the beat.

The sinus node is located at the intersection of the superior vena cava and the right atrium laterally. While the sinus node cells have an intrinsic automaticity of their own, they are influenced by the sympathetic and parasympathetic nervous system,. The former increases the heart rate during exertion or when the sympathetic tone is high while the latter slows the heart rate.

So as you might guess, the cardiac action potential and regulatory mechanisms lies within the heart itself and depends on these intrinsic nodes rather than the CNS.

When the failing heart is removed during a transplantation and the heart loses its CNS innervation, the heart still beats, but the contraction rhythm is uncoordinated, owing to the automatic electrical impulses of the pacemaker cells.

Comminuted fracture from AK-47!
This is the kind of damage an AK-47 can cause.
This patient was shot in the right leg by an AK-47 assault rifle at close range.
It looks as if the bullet(s) directly hit the proximal femur, shattering it in the process, spreading ballistic fragments throughout the soft-tissues of the thigh. This kind of damage will warrant a trip to the OR for an intramedullary rod for fracture fixation.
