Please see the attached instructions to sign up for the flu clinic on Friday, October 9th.

HOW TO SIGN UP YOUR CHILD FOR SCHOOL FLU CLINICS:

1) Go to wasatchcountyhealth.gov and click "School Flu Clinics" under popular resources section of the homepage

2) Click on your school name and then choose (Flu Shot or Flu Mist) under "Appointment Reason"

3) You will not need to select a date or time. Instead just "Send Request" to register for the clinic

4) Watch your text messages and email for appointment confirmation details and an electronic consent form
