Active Day Families - if your loved one is on a waiting list for HCB or MPW waivers, you will need to follow this process to maintain status on the waitlist. If you have not received a copy of the letter in the mail, we do have copies at the center. Our nurse can assist with obtaining physician signatures. Please reach out our center director at 502-875-7555 with any questions or for assistance.

ACTION REQUIRED: MPW & HCB WAIVER WAITLISTS

If you or a loved one is currently on the Michelle P. Waiver (MPW) or Home and Community Based (HCB) Waiver waitlist, watch the mail for an important letter from the Kentucky Cabinet for Health and Family Services (CHFS).

Complete the Provider Attestation Form, have it signed by an approved provider, and mail it by January 31, 2027. Failure to submit the form by the deadline may result in removal from the waitlist.

Call DCBS at 855-306-8959 to make sure the mailing address on file is correct.

This requirement applies only to people on the waitlist. It does not affect current waiver recipients who are already receiving services.
