Call for Abstracts: 12th Annual Transgender Medical Conference

The South Florida Transgender Medical Consortium (SFTMC) is calling for abstracts for the 12th Annual Transgender Medical Conference, to be held at the University of Miami Alumni Center in Coral Gables, Florida, on September 21-22, 2023.

This year's conference theme is "Supporting Families," and we are seeking abstracts on all aspects of transgender medicine, mental health, substance misuse, HIV, and social work. We are particularly interested in abstracts that address the following topics:

The impact of recent legislative action in Florida on transgender people and their families
Innovative approaches to providing transgender healthcare in a hostile political climate
Strategies for supporting transgender families during times of crisis

Abstracts should be submitted online by June 1, 2023. For more information and to submit your abstract, please visit www.sftransmed.org.

The recent legislative action in Florida is a direct attack on the rights of transgender people. These laws are harmful and discriminatory, and they put transgender people and their families at risk.

The SFTMC is committed to providing medical teams with the education needed to provide transgender people with the healthcare they need and deserve. This conference will bring together a diverse group of experts to share their knowledge and expertise, and to develop strategies for supporting transgender people and their families during this challenging time.

Opening Session and Keynote: Lucie Fielding, MA Author: Trans S*x-Clinical Approaches to Trans S*xualities and Erotic Embodiments

Description: All too often the focus of gender-affirmative medicine is the alleviation and resolution of gender dysphoria. Such a focus has the effects of limiting the clinical imagination and foreclosing conversations around pleasure-positive embodiments. But what if resolving gender dysphoria wasn’t the point? This address, emerging from both the lived and the professional experiences of a trans s*xual health professional, will challenge attendees to re-center gender-affirmative clinical practice around “gender-pleasure.” Attendees will emerge from this address with frameworks and strategies to enliven their practices and better support trans and non-binary patients around finding pleasure in their bodies.

Learning Objective (1): By the end of this address, participants will be able to define the concept of “gender-pleasure” and demonstrate an increased level of comfort and confidence in supporting trans erotic embodiments within clinical settings.

Bio: Lucie Fielding, PhD, MA, LMHCA, Resident in Counseling (she/they) is a q***r, non-binary femme, and a therapist practicing in Virginia and Washington. She holds an MA in Counseling Psychology at Pacifica Graduate Institute (2018) as well as a PhD in French from Northwestern University (2008), where she specialized in erotic literature. Their background in literature attunes them to the many ways that cultural scripts inscribe themselves on our bodies and can inform our embodied erotic lives. In addition to their work as a therapist, Lucie teaches in the S*x Therapy Certificate Program at Antioch University-Seattle. They are the author of Trans S*x: Clinical Approaches to Trans S*xualities and Erotic Embodiments (2021), which was named a finalist for a 2022 Lambda Literary Award (Lammy) in the Transgender-Nonfiction category. Current Florida Legislation Regarding LGBTQ+ Youth and Gender Affirming Care: Strategies for Clinician Advocacy: Natasha L. Poulopoulos, PhD, Jonathon Poquiz, PhD, and Lisa Gwynn, DO, MBA, MSPH, FAAP.

Coming off the heels of the ‘Don’t Say Gay’ law and multiple political attacks across the country on LGBTQ+ youth, the Florida Department of Health (FLDOH) released guidelines opposing all forms of gender affirming care for the treatment of youth gender dysphoria, including social affirmation and transition. The FLDOH cited few outdated, methodologically flawed research published in low-impact journals and an opinion piece that contrast formal medical guidelines and ignore an entire body of research; more commonly referred to as cherry picking data. Multiple studies demonstrate improved mental health outcomes for youth, including reduced suicidality, after receiving gender affirming care (social transition, pubertal suppression, and/or hormone treatment). The panel discussion will focus on brief status of FL legislation regarding gender affirming care and strategies clinicians can take to advocate for LGBTQ+ youth and gender affirming care.

Objectives
Attendees will be able to: 1) identify current Florida legislation and policies focusing on gender affirming care and 2) LGBTQ+ youth and develop strategies for advocacy within their community, organization, and statewide.

Biographies: Natasha L. Poulopoulos, PhD is a pediatric psychologist at Holtz Children’s Hospital in affiliation with the University of Miami Miller School of Medicine.

Jonathon Poquiz, PhD is a pediatric psychologist and clinical director of the Gender Affirming Care Clinic at Johns Hopkins All Children’s.

Lisa Gwynn, DO, MBA, MSPH, FAAP is an Associate Professor of Clinical Pediatrics and Public Health Sciences and Interim Division Chief of Child and Adolescent Health at the University of Miami Miller School of Medicine.

Utilizing Culturally Responsive Interventions When Working with Parents of Gender Nonconforming Children: Tatiana Escandon, PhD, LMHC, CST

Gender nonconformity concerns in children continues to be an area of study that is full of misinformation, stigma, and judgment. Most professionals do not possess enough training to guide parents and children in navigating the challenges that gender nonconformity can bring, which is critical because education, family support, and acceptance are vital in the well-being of gender nonconforming individuals as they prevent su***de and other health disparities, especially of those holding minority identities. Grounded in the gender affirmative model integrated with intersectionality, clinicians will learn culturally responsive research-based interventions that have been found to have a positive impact on the lives of gender-diverse individuals and their families. Mental health providers will also learn about the importance of being culturally responsive and ways to achieve it.

Course Objective #1: The mental health provider will be able to identify their own intersecting identities.

Course Objective #2: The mental health provider will be able to describe the emotional experiences of parents of gender nonconforming children.

Course Objective #3: The mental health provider will be able to name some of the existing strength-based family interventions.

Biography: Tatiana Escandon, Ph.D., LMHC., CST (She/Her/Ella) is a proud q***r Latina who has devoted the past 10 years of her life to the study of gender, s*xuality, human relationships, and the relationship of culture and other intersecting identities. Dr. Escandon facilitates post graduate level trainings, conducts consultation groups for clinicians on LBGTQ+ topics and provides therapeutic and coaching services. Best Practices for a Trans Affirming Behavioral Healthcare Treatment Experience: Trauma Informed Care Approaches - Donna Weinberger CEO/Founder Inspire Recovery & Jaki Neering, LCSW, Program Director

The session will explore the application of a trauma-informed approach to providing transgender affirmative care to the trans community. Through our direct client care experience, we will teach you the standard of care necessary to serve the transgender community in addiction and mental health treatment facilities. We will discuss in detail the trauma of gender dysphoria and how to treat the trauma of gender dysphoria. We will outline the life-saving interventions paramount to serve the trans community. These will include medical interventions, family interventions, and social interventions. You will learn the do’s and dont's when working with transgender individuals. The goal is to leave this workshop with a cultural shift that is pertinent to providing transgender-affirming care.

Donna Weinberger, CEO, is at the forefront of behavioral healthcare for LGBTQIA+ individuals by challenging age-old traditional systems. Donna encourages their staff by providing continuing education to commit a standard of care for the LGBTQIA+ community with addiction through trauma-informed LGBTQIA+ affirming, life-saving interventions.

Jaki Neering, LCSW has a loving & progressive approach working with the LGBTQIA+ community. Jaki graduated from Barry University in 2016 with a Master of Social Work degree. Jaki believes in the power of trauma-informed interventions, which recognizes human vulnerability but focuses on finding strengths, resources, and capacity for healing and recovery by promoting factors that will enhance a person’s resiliency.

Gender Affirming Care Clinic at Johns Hopkins Children’s Hospital: Model of Care and Role of Behavioral Health: Jonathan Poquiz, PhD & Allegra Kartha, MSW

As behavioral health providers, we will provide data on the psychosocial outcomes of trans and non-binary youth initiating care within the Gender Affirming Care Clinic. Given that we are a pediatric institution, we will be solely focused on discussing populations of trans and non-binary youth and adolescents.

We will describe health disparities and barriers to gender-affirming care that disproportionately affect transgender and non-binary youth (TNB) and describe a clinical model of care that emphasizes an interdisciplinary approach in providing care. We highlight our clinical model by emphasizing the role of behavioral health in providing psychoeducation on the gender affirmative approach to care for all TNB youth and their families initiating care. Interdisciplinary collaboration and utilization of behavioral health providers can address barriers in accessing care by being the first point of contact for families initiating care.

Dr. Poquiz (he/him) is a pediatric psychologist in the Gender Affirming Care Clinic at Johns Hopkins All Children’s. He specializes in providing outpatient and interdisciplinary care for transgender and gender diverse youth, as well as for youth with differences in s*x development/inters*x diagnoses. His research is centered on these populations.
Allegra Kartha (they/them) is the social worker for the Gender Affirming Care Clinic at Johns Hopkins All Children’s Hospital. They joined the hospital staff in 2021. In their clinical work, Allegra utilizes a a non-judgmental approach pulling from different interventions.

Addressing Bottom Dysphoria when Treating Transmasculine, Gender Non-Binary, Andro (male) Identified, Patients in Psychotherapy: David Wohlsifer, PhD, LCSW & Caidin Smith, MSW Candidate

“Bottom Dysphoria'' is a term often used to describe a person’s discomfort with their ge****ls, particularly when their ge****ls do not match their gender identity. Bottom dysphoria can be a common presenting concern for many transgender and gender non-binary (TGNB) people who seek psychotherapy. One study found that 71% of non-binary AFAB individuals and 46% of transmen did not want bottom surgery (Tristani-Firouzi, et al.,2021). There are a myriad of reasons why some AFAB TGNB people do not seek bottom surgery. Though someone may not seek bottom surgery, it does not necessarily mean they do not experience bottom dysphoria. Bottom dysphoria is a complex and dynamic issue, each person’s experience of it can be different, and it is often associated with feelings of loss and frustration. Considering the issues raised above this presentation will discuss affirming methods of addressing bottom dysphoria in AFAB TGNC people in psychotherapy. Emphasis will be placed on applying the framework of Pauline Boss’s Ambiguous Loss theory (Boss, 1999) to addressing bottom dysphoria when working with AFAB TGNC clients.

At the close of this webinar participants will be able to:
1.Apply the theory of ambiguous loss when treating bottom dysphoria in AFAB TGNB clients in psychotherapy.
2.Identify approaches to address bottom dysphoria when treating AFAB TGNB clients.
3. Discuss the complexities surrounding the decision to get bottom surgery from a biopsychosocial perspective.

Transgender Client Competency: Cole Timonere, JD, MA, NCC

The transgender population remains one that is often overlooked in the counseling and medical communities (Chang et al., 2019; Dierckx et al., 2017; Edwards et al., 2018; Giammattei, 2015). This is especially troubling when taking into consideration that 75% of transgender people have previously sought out counseling (often in furtherance of surgical procedures), and an additional 14% intend to seek out counseling in the future (Budge, 2015).
Transgender individuals often present with a myriad of issues due to their marginalized place in a predominantly binary society. Unfortunately, many within the population are left without support due the often-strained relations present amongst transgender individuals and their family and friends (Hall & DeLaney, 2019). Thus, the population is more inclined to turn to professional counselors for empathy, guidance, and support in their challenging journey. And further, to medical providers for assistance in an affirmation process, should that be the route that it most fitting for the client.

In order to provide gender affirming quality care to transgender individuals, clinicians and medical providers must have knowledge about transgender specific needs, they must understand the unique and individualized challenges that transgender clients face and be prepared and willing to provide support to their clients in a society that is ultimately transphobic and antagonistic. This presentation will serve as a practical guide toward that end.

Cole Timonere is a PhD student studying counselor education and supervision. He is an Assistant Clinical Director at Recreate Life Counseling and has a private practice devoted to working with folks who identify as gender expansive. Cole began his career as an attorney, however, shifted to counseling when he found that his passions lie in assisting folks in a different capacity than could be done utilizing the law. Cole is devoted to creating an inclusive and affirming experience for his clients.

Nikole Parker is an Orlando native of Black, Puerto Rican & Italian descent. At the age of 19 she left her job and childhood home to begin her journey to living authentically. After 3 years of black-market hormones and unsafe lifestyle practices, she came back to Orlando to begin her next chapter. Nikole’s passion is advocating for human rights, protections for all and the importance of everyone having a seat at the table.

Nikole is the Director of Transgender Equality at Equality Florida. Nikole is also the Co-Founder, Director of Operations for the Gender Advancement Project (GAP) and sits on the board of The Center Orlando.

She sat on various non-profit board of directors &/or advisory councils including the onePULSE Foundation Memorial Task Force, Orlando United Assistance Center, Contigo Fund, Spektrum Health, Peer Support Space and the Central Florida LGBTQ+ Relief Fund. Nikole was also recognized as one of Watermark's Most Remarkable People of 2018 and with the Humanitarian of the Year award that same year. In 2019, Nikole was nationally recognized by Congressman Darren Soto in honor of LGBTQ+ Pride Month and her biography was read into the Congressional Record. In 2020, Nikole was honored with the Hope & Help Hero Award. In 2021, she was awarded the Emerging Leader Award from the Association of Fundraising Professionals.

Through advocacy Nikole hopes to encourage open dialogue on gender issues, state and federal protections for all and linkage to proper transitioning resources. Her hope is to teach people that the black-market way of transitioning isn’t the only way.

This training provides a foundation rooted in mental healthcare and education for those whose loved one just came out as Trans*. Whether their child is 18 months, 18 years, or 80 years, this is for anyone who is in a parental or guiding role for someone who identifies as Trans*.

Learning Objectives: 1) To calm parents and to offer guidance, 2) to provide support that is rooted in best practices, kindness, compassion, and affirmation of their trans* loved one, and 3) to introduce foundational knowledge both on the common experiences of a child coming out to their parent/guardian so that intentions about responses can be prepared for by audience members

Presenter bio: With over 25 years of experience guiding the world’s top leaders in business, education and community via individual, small group and full-staff trainings, Dr. Kryss Shane LSW, LMSW (she/her) has been featured as America’s go-to Leading LGBT+ Expert in The New York Times, ABC News, Yahoo!, and CNN. Her writing has also appeared in the Journal of Nonprofit Education and Leadership, Huffington Post, International Council of Professors of Educational Leadership, The New Social Worker Magazine, and many more.
Shane is known for helping businesses address their unique Diversity, Equity, and Inclusion needs in an approachable, manageable and actionable way. This includes alterations to physical spaces, updating hiring practices, re-drafting policies and procedures and more. Shane holds two master’s degrees, two licenses to practice mental healthcare, and she is currently serving as a Lecturer at Columbia University and an Adjunct Professor at University of Massachusetts Global, Fordham University, and Northern Louis University.

Medical providers and allied health professionals are observing a notable increase in cases where clients self-disclose their gender identity. Clients navigating their authentic experience with gender often contend with other challenges such as anxiety, depression, and self-harm ideation, barriers to care in medical systems, and psycho-social factors including family or peer rejection. Even if family members are supportive, clients can find themselves in unsafe school or work environments with limited support systems, situations that can be categorized under minority stress theory. This session will review the current state of knowledge regarding best practices with transgender and gender nonbinary clients. A young adult speaker who has received gender affirming medical care in the past will share their personal experiences and engage in Q&A with participants. The importance of linking clients and their families to peer support and educational services will also be covered. Vital referral contacts and resources across South Florida will be shared.
