Please join us in welcoming Yolanda to the Family Planning Services team!

Yolanda joins us through the Pathways HUB at LCCAA Lorain County Community Action Agency as a Community Health Worker supporting pregnant and postpartum moms. Her passion for this work comes from her own experience with a Community Health Worker after the birth of her daughter 11 years ago.

“I believe every mother deserves access to compassionate care and resources during their journey.”

Yolanda has been a certified Community Health Worker for six years and is excited to bring her experience, compassion, and connections to the FPS community. On her days off, she loves spending time with family, shopping, watching movies and listening to music.

Know a pregnant or postpartum mom who could use support? A referral form can be completed by the mom herself or by someone who knows a mom in need.
