If your child was remote during the 2020-2021 school year, please complete the form below by Wednesday, June 30 to help us prepare for the upcoming school year.

This form is for students who were remote during the 2020-21 school year. Please complete this form by Wednesday, June 30th to ensure that your child is added to a bus route in August (FALL 2021). Thank you!
