Community Advocate / Case Manager (Per Diem)

Join a growing Sarah Care Medical Adult Day Health & Adult Foster Care team in Massachusetts dedicated to helping seniors and families navigate healthcare, social services, and community support.

We are seeking compassionate, motivated, and professional individuals who are passionate about making a real difference in the community.

Requirements

Bachelor’s Degree required
Experience in case management, healthcare, social services, or community advocacy preferred
Must be organized, caring, and able to work independently
Multilingual candidates strongly encouraged:
Vietnamese
Chinese
Cambodian/Khmer
