Momentum Daycare is an extended ministry of Momentum Church located in Crown Point at 2200 N Main St. Crown Point. We are committed to providing high quality child care to families in the surrounding communities. While children are away from their families they will be cared for in a safe, nurturing and positive environment. Our staff is committed to caring for children in a loving and consistent manner. Hours of Operation and Ages Served
Momentum Daycare operates from 6:00 a.m. to 5:00p.m. Monday through Friday. Hours or days varies depending on Holidays and Emergencies. We care for children ages 6 weeks to 5 years old in age. Items Needed for Enrollment
In order to enroll the following items are needed:
• Birth Certificate
• Current and Updated Immunization Records
• Recent Physical Exam (within the last 6 months with doctors signature)
• Copy of Parent/Guardian License/State Photo I.D.
• Registration and Child Care fee are due before service begins
*Momentum Daycare requires 24-48 hours advanced notice for new children. Enrollment is based on availability in age group and/or classroom. Our Teachers:
