If your student still needs a backpack for the upcoming school year, be sure to check out this community event hosted by the Mackenzie Kendall Memorial Foundation and Sonic Drive-In of Coffeyville!

📚✏️ USD 445 School Supply Assistance – Now Open! 🎒

If your child is in Kindergarten through 12th grade and your family could benefit from school supply assistance for the upcoming school year, please complete the request form below.

⏰ Important Deadline: The school supply request forms will close on Monday, August 10 at 4:00 PM. No requests will be accepted after this deadline, so please be sure to complete your form before then.‼️

📝 Important Information:
• Complete one form for each child needing school supplies.
• School supplies are available for Kindergarten–12th grade students.
• Early Learning Center (ELC) supplies are not available through this program this year.
• CES (K–5): We have a limited number of gender-neutral backpacks available (while supplies last). If your child needs a backpack, please select "Yes" on the request form. School supplies and backpacks (if requested and available) will be delivered to your child's classroom by Meet the Teacher through CES Community Health Worker Hailey Collins.
• RMS & FKHS (Grades 6–12): Students will receive their school supplies on the first day of school from Community Health Worker Esther Cuevas.
• Please note: This program provides basic school supplies. Some classroom-requested items (such as tissues, Clorox wipes, headphones, and other specialty items) are not included. Please review the request form for the complete list.

Si su hijo está en el kinder hasta el grado 12 y su familia podría beneficiarse de la asistencia de útiles escolares para el próximo año escolar, complete el formulario de solicitud a continuación.
