Does your child have a food allergy or medical need that requires a special diet at school? Please make sure Child Nutrition has your child’s current dietary accommodation paperwork on file.

📋Follow the link to print the Physician’s Request for Special Dietary Accommodations form for any new dietary accommodation.

🔄 Has something changed? The same form can also be used to change, update, or discontinue an existing dietary accommodation.
