What is the Medicare GUIDE Program?
GUIDE stands for "Guiding an Improved Dementia Experience," a Medicare program created to support individuals living with dementia and the family members caring for them.
The program is designed to improve quality of life through services that may include:
đź’š Dementia care coordination
đź’š Caregiver education and support
đź’š Access to respite care services
đź’š Ongoing guidance and resources throughout the dementia journey
For eligible individuals, respite care benefits may be used to support programs such as adult day services, helping caregivers receive meaningful support while their loved one remains engaged in a structured and person-centered environment.
I am excited to share that I am now a certified Medicare GUIDE Assessor for HOPE Alzheimer’s & Dementia Care.
If you have a loved one living with dementia and would like to see whether they may qualify for participation through our HOPE program, I would welcome you to reach out. I can complete a GUIDE assessment specifically to determine eligibility for services through HOPE Alzheimer’s & Dementia Care. Please call to learn more or schedule an assessment.

My heart is full as I share something very special happening at HOPE Alzheimer’s & Dementia Care.
Some time ago, we started what we call our MEMORY MAKERS FUND, a sponsorship program created to help seniors and families who may be experiencing financial hardship but would deeply benefit from meaningful engagement and attending our program.
Today, we are honored to share that our first sponsorship has already been given.
Because of the generosity of one incredible individual, one of our guests was able to continue receiving support, companionship, purpose, and wonderful moments during a season when attendance may not have otherwise been possible.

I am excited to share that HOPE Alzheimer’s & Dementia Care is now officially participating in the GUIDE Program through the Centers for Medicare and Medicaid Services, in partnership with Continuum Care Partners and Empower Me Medical Group as part of an integrated dementia care support network.
We are also proud to be the first social adult day program in Butte County to participate in the GUIDE Program for Alzheimer’s and dementia care.
The GUIDE Program is designed to support individuals living with dementia while strengthening caregiver support, improving coordination of care, and helping individuals remain safely at home with a higher quality of life.
At HOPE, we are now accepting GUIDE assessments by appointment on Tuesdays and Thursdays. If you would like to explore whether your loved one may qualify, please reach out directly to schedule an assessment.
General eligibility includes:
• Enrollment in Traditional Medicare
• A confirmed dementia diagnosis such as Alzheimer’s disease, Lewy body dementia, frontotemporal dementia, or vascular dementia
• Medicare Parts A and B with Medicare as primary coverage
• No enrollment in Medicare Advantage, Special Needs Plans, or PACE programs
• Not currently receiving Medicare hospice benefits
• Not residing in a long-term nursing facility

JUST WORDS TONIGHT…
One of our guests stood up from our luncheon table today, walked over to me in the middle of preparing lunch for everyone, and said something I will never forget.
“I just want you to know… I see you. I see all the hard work you’re doing, and I know it’s not easy. I’ve done similar work. I really think you’re doing a great job, and I’m so thankful for you.”
I stood there completely speechless.
Moments like this are such a powerful reminder that dementia does not erase a person’s heart, emotions, compassion, or ability to connect. There is still awareness. There is still love. There is still deep human connection.
For someone to look at me with such sincerity and recognize the care, time, patience, and love that has gone into this journey over the years… it touched me in a way I cannot fully put into words.
I instantly broke down in tears, then quietly gathered myself again.
Not out of sadness, but because in that moment, I felt truly seen. Maybe for the first time in such a deep and meaningful way since beginning this journey with HOPE.
It reminded me exactly why I do what I do.
Behind every diagnosis is a person who still feels valued, who still notices kindness, who still gives love and encouragement to the people around them every single day.
Today was one of the most meaningful moments I have ever experienced, and I will carry it with me forever. ❤️
