Sinai Chicago is working with the Alliance for Health Equity (Alliance), a group of over 30 hospitals, local health departments, and community organizations in Chicago and Suburban Cook County, to develop the 2022 Chicago and Suburban Cook County Community Health Needs Assessment (CHNA). The CHNA helps us understand the health status of communities across the Chicagoland area and provides opportunities for residents to provide input on effective strategies to address priority health needs (the 2019 CHNA can be found here). Information gathered through this process will also be used to inform the 2022 CHNAs for Mount Sinai Hospital, Holy Cross Hospital, and Schwab Rehabilitation.
One way we plan to obtain community input is through a Community Input Survey. I would like to invite you to take and share the anonymous five-minute Community Input Survey with members in your community.
The survey is intended for residents of Chicago and Suburban Cook County and is open to all ages. To participate in the survey please visit http://tiny.cc/AHECHNASurvey. Please complete the Community Input Survey by December 1, 2021.

Rental Assistance:
During this special application period, NHS will accept NEW applications for Chicago’s Emergency Rental Assistance Program. Financial assistance is available for income-qualified landlords and tenants who have experienced a financial hardship due to the covid-19 pandemic. The program is open for tenants and landlords until funds are exhausted or through Tuesday, December 14, 2021.
The program will provide up to 12 months of unpaid rent and utilities and up to 3 months of future rent and utilities.
