Hello!
We'd like as many of your families to take the family survey as possible before they start getting books. It is meant to be taken by a primary parent or guardian of the child. If there are multiple children in the same family, we ask that they fill out a survey for each child. Please work with your CCP coach to make a survey collection plan to get as many responses as possible.
