It is our responsibility as members of the interprofessional care team to ensure that each patient with HF is managed appropriately according to the best available evidence, regardless of our role." - Barbara "Bobbi" Leeper RN, MN, CNS M-S, CCRN, FAHA

In order to be effective leaders, case managers (CMs) need written and verbal communication skills and the capacity to form relationships with patients, co-workers, and colleagues.

This includes the ability to influence, interview, and negotiate.

Case management also requires listening, presentation and public speaking skills along with the ability to share information effectively to diverse groups.

Case managers use information to develop plans and to identify critical issues. Critical thinking skills facilitate CMs' ability to understand instructions and policies and to evaluate information and situations in order to make independent decisions.

"To appropriately address the vast dimensions of domestic abuse, intervention is warranted at the micro and macro levels of practice. This involves assessment of and advocacy for not only patients and their circumstances but also the legislation mandating requisite resources and funding.

Case managers are often on the front lines of contact with abuse victims. Their blend of biological, psychosocial, and behavioral clinical expertise finds them uniquely well suited to the role of advocacy for this population especially."

Attitudes towards individuals with addiction are heavily moralized. This may result in both self- and other-labeling and, consequently, the development of a "spoiled" public identity.

CMs and other interprofessional care team members can provide education and leadership in destigmatizing addiction across the entire continuum of care.

Ultimately, although there is no universally established model, an individual CM may follow a patient along the spectrum of care.

This differs from the more common system where CMs coordinate care with other CMs across each level of care. A term which might be applied to an individual CM focusing along the entire spectrum of care is longitudinal case management.

This type of case management is appropriate for patients who are determined to be high risk or high complexity using a variety of scoring, screening and assessment tools and for whom an "endpoint" isn't clear.
