1. Disability Forms for emotional/psychological conditions with or
without medical/physical conditions. For college or work
settings such as reasonable accommodations.
2. Medical Leave of Absences- in collaboration with primary MD
if coexisting medical/physical conditions. For example,
depression sets in after a new medical diagnosis (i.e. auto-
immune, cancer, parkinsons, alzheimers)
3. Letters of Support- You need a professional to attest to
a need.
4. Letters of Verification- You need a letter verifying attendance.
5. Adult Outpatient Psychotherapy (Mental Health) Sessions-
short term or longer term - 3 months up to 1 year
6. Online Educational Program-Good Vibrations Building Family
Resilience through Coparenting; Group Class or Private
7. Child Custody Written Evaluations (730, 3118, 3111)-
Collaborate with Family Law Attorneys, Law Enforcement,
DCFS, Probation, other Agencies/Persons
8. Mental Health Written Evaluations - for Court or for
assessment. Also second-opinion assessments if you
doubt or have issues with a prior MH evaluation.
9. Marriage Counseling- Prior to Marriage or if having
current difficulties.
10. Couples Counseling- To strengthen or improve relationships.
11. Family Counseling- Consisting of 3 or more family members
that are willing to work on specific issues and for a limited
time as agreed to prior to start.
12. Court-Ordered Reunification Counseling between estranged parent and child;
13. Employee Assistance Program -Free benefit paid by employers for limited number of sessions.
