Please see the information below around liability waivers that are required for all participants.
Effective August 1, 2026:
All participants using Next Level Sports are required to have a completed liability waiver on file BEFORE participating in any activity inside our facility.
This applies to all participants, including children, athletes, teams, and guests using the facility for practices, lessons, rentals, open play, parties, or other activities.
If you have not already completed a waiver, please visit the link below in the comments or scan the QR code to complete it before participating.
One waiver per participant is required.
For your convenience, printed copies of the waiver QR code are also available on the tables at the facility. Simply scan the QR code with your phone to complete the waiver.
Please help us keep the process quick and easy by taking care of this before arriving or before beginning your activity.
