-We will review the 3608 (IPC) form from HHSC, discuss roles and responsibilities of the LIDDA vs. the Provider concerning the IPC at enrollment, transfer, renewal, and any revision.

-We will look at sample IPC’s and review each component of the form to ensure that you understand how to complete the form.

-We will discuss when and why revisions are done and procedures to follow when a revision or renewal needs to occur.

-We will also touch on some important things to remember and tips when entering renewals or revisions into TMHP.

This session provides practical guidance for admitting individuals through new enrollment or transfer, including key pre-admission and admission activities. It will also cover required initial assessments, such as the Comprehensive Nursing Assessment, along with evaluations for special needs, protective devices, and restraint risks. Additional topics include nursing-specific considerations, meeting individual needs, monitoring and documenting services, ensuring access to waiver and non-waiver services, and the nurse’s role in training staff who work directly with individuals. Although the training is relevant for all team members involved in health. service planning and implementation, it is intended primarily for RNs.
