Fun 2-rooted canine with long, calcified roots. The tooth supports a partial denture that has been uncomfortable to wear for the last week to due a developing apical infection in the buccal root.

Love the look of instant Strindberg healing! Nice Emory Prof presented with an expanding apical lesion that was getting concerningly close to his implant-supported bridge. Didn’t want to mess around here. Pack & wack w FDBA & col membrane. A chance to cut is a chance to cure in this case. Internal contents appeared cystic with a nice heavy dose of purulence within the sac. Very satisfying to enucleate. Should do well. Patient is on a heavy dose of eliquis that his cardio advised he stay on, so the site was a little “wetter” than I’d like.

Here in Buckhead we have an older patient population (at least in my practice), so almost every tooth is severely calcified. In this case the roots were completely obliterated and there was zero evidence of a canal space on the CBCT scan and X-rays. Add to the complexity the fact that this is a premolar with naturally very thin, delicate roots. These cases are like a histological study in which I try to read the subtle differences between tertiary and radicular dentin and letting those dentinal maps carefully guide me into the root until a canal is found. This is painstakingly precise work, but it keeps me on my toes.

What type of root canal would you want for yourself or a family member? It’s been 10 years since I’ve become minimally-invasive by down-sizing my armamentarium. Minimally-invasive techniques has been the single greatest advance in Endodontics, full stop. Nothing impacts tooth survival more in our field. Endo used to be the kiss of death for the tooth, but that’s no longer the case. Unfortunately, 10 years later, more than 95% of the root canals done by both endodontists and GP’s continue to use traditional access and shaping concepts with large, aggressively tapered rotaries (sad for patients). Here are some older lecture slides I made on the impact of small changes in tooth preservation. Thank you to John Khademi, David Clark, and Eric Herbranson for developing these concepts and these files back in 2012.

Great to see the healing at 6 months postop. We opened the tooth initially and found it was not cracked, so this gave us the confidence to move forward with treatment.

Tricky tricky. 2-1-2 anatomy in a 27 mm P root. It was tough getting the cones to seat into the deep apical split since the root was so long and the shapes and access so conservative. Ended up custom rolling my own cones to make them less tapered so they could fit to conservative shapes, and used endo ice to freeze the cones to hold the curved shape I needed to seat them into the respective branches.
