AMS Athletic Department has partnered with FinalForms, an online forms and data management service. We are moving away from RMA to streamline the compliance transition between AMS and Paul Blazer HS.

All students planning to play MS sports must create accounts in FinalForms and register your student(s), as well as UPLOAD PHYSICALS ONLINE. While listed as optional in your account, it is important to upload physicals virtually. We will not file hard copies or have them sent to the school, so you can always keep physicals at home when needed.

Take a picture with your cell phone of the physical, and upload!

FinalForms allows you to complete and sign athletic participation forms for your students. The most exciting news is that FinalForms saves data from season-to-season and year-to-year, meaning that you will never need to enter the same information twice! FinalForms also pre-populates information wherever possible, for each of your students, saving you time.

OVC CHAMPIONS & OVC RUNNER UPS!! Boys win out over Rowan County 1-0 for the gold. Girls finish in 2nd, losing to Russell in a penalty shootout. Congratulations to our boys and girls soccer programs!
