The Louisiana Public Health Institute (LPHI) is conducting a Patient Experience Study to better understand how women access and experience healthcare before, during and after pregnancy:

If you're currently pregnant or have given birth in Louisiana within the last 3 vears, vour voice can help shape the future of maternal healthcare in our state.

✨️Complete a brief online survey for a chance to win a $100 gift card.
✨️Selected interview participants will receive a $50 gift card.

You may qualify if you:
✔️Are 18 or older
✔️Are currently pregnant or gave birth in Louisiana within the past 3 years
