Hey WINTER ATHLETES!!! It is that time of year!!! Time to get your FinalForms accounts set up and/or update the information.

Go to affton-mo.finalforms.com to set up or log into your account.

If you are a new athlete to Affton High School, a Parent or Guardian must set up your account first. Once the account is set up, all forms must be completed and a CURRENT PHYSICAL must be uploaded. After the parent section is completed, there are student signatures that are required. An email from FinalForms for your signature, will be sent to your student email account upon completion of the parent section.

For returning athletes, all items must be updated yearly. Physicals may be 2 years, however don't assume yours is or that you have have time to spare!!! CHECK YOUR ACCOUNT. Make sure all information is in GREEN. Red or Yellow needs attention.

Get your account up-to-date TODAY!!! Make sure you have a CURRENT PHYSICAL uploaded.

You will not be eligible for tryouts until your paperwork in FINALFORMS is completed.
