Responsible AI at JPER: Empowering Pracademic Synergy

Artificial intelligence is reshaping research, but ethical integrity remains uniquely human. JPER formally outlines its AI Policy to ensure transparency, accountability, and the enhancement of "pracademic" synergyโ€”bridging frontline emergency experience with rigorous academic standards.

Key Guidelines for Authors:
Human Accountability: AI tools can assist in writing, editing, or data analysis, but cannot be listed as authors or co-authors.
No AI Citations: Generative AI must not be used as an information source or to create unverifiable data and references.
Full Disclosure: Any AI utilization must be explicitly disclosed in the manuscript (Methods or Acknowledgments), detailing the tool name, version, and specific purpose.
Reviewer Confidentiality: Peer reviewers and editors are prohibited from using external generative AI to draft or summarize evaluations to safeguard confidentiality.

Who let the dog out?
Lessons from a Fatal Dog Bite-Induced Femoral Artery Rupture

A critical case study published in JPER details a tragic incident where a 56-year-old male suffered a fatal femoral artery rupture following a bull terrier bite, leading to traumatic Out-of-Hospital Cardiac Arrest (OHCA).

Main Findings:
โ€ข Critical Injury: The victim sustained a 15cm x 10cm open wound to the proximal thigh, causing massive hemorrhage.
โ€ข Pre-hospital Intervention: EMTs applied dual tourniquets and performed advanced life support, including Lucas device compressions and Epinephrine administration, but the patient could not be resuscitated.
โ€ข Future Recommendations: The study emphasizes the need for advanced hemostatic dressings, improved trauma training for massive vessel injuries, and public education on early hemorrhage control.

ใ€Survival Against the Odds: Prehospital Needle Decompression Saves a Lifeใ€‘ ๐Ÿš‘๐Ÿ’จ

Can a single clinical decision on the street reverse a fatal "Pulseless Electrical Activity" (PEA)? A new case study in JPER by Chi Mei Medical Center and Tainan City Fire Bureau proves it can.

๐Ÿ“ The Turning Point:
A 52-year-old motorcyclist rapidly deteriorated into traumatic arrest. Recognizing subcutaneous emphysema and diminished breath sounds, EMS providers suspected Tension Pneumothorax causing obstructive shock.

โœจ The Result:
Immediate Needle Decompression led to the Return of Spontaneous Circulation (ROSC). The patient was later discharged with a perfect neurological outcome (CPC 1).

๐Ÿ’ก Key Takeaway:
The study advocates for the "HOTT" mnemonic (Hypovolemia, Oxygenation, Tension pneumothorax, Tamponade) over standard ACLS for trauma, emphasizing that rapid identification of reversible causes is the "Art of Saving Lives."

ใ€JPER Research: Impact of Organizational Climate on Workplace Bullying among Firefightersใ€‘
Workplace bullying significantly affects mental health and operational performance. This study published in the Journal of Paramedicine and Emergency Response (JPER) by researchers from National Taiwan Normal University investigates the correlation between organizational climate and bullying experiences among 733 firefighters using Ordinal Logistic Regression.
Key Findings:
โ€ข Prevalence: 12.7% of participants identified as bullying victims, while 34.8% reported occasional negative acts.
โ€ข Protective Factors: Higher scores in Employee Involvement (OR=0.698), Supervisory Support (OR=0.633), and Effort (OR=0.649) were associated with a significantly lower risk of being bullied.
โ€ข Risk Factors: High Pressure to Produce (OR=1.841) significantly increased the likelihood of experiencing workplace bullying.
โ€ข Management Strategy: Enhancing communication channels and psychological support systems is essential for a healthier professional environment.
Takeaway: Reducing organizational pressure while fostering supervisor support and employee participation are critical steps in preventing bullying within fire services.
