CHILD:
• 2 Passport-size photos.
• Computerized Birth Certificate (Copy and Original).
• Immunization Card (Copy and Original).

PARENT:
• 1 UTILITY BILL (Copy and Original).
• 1 VALID FORM of ID. (Copy and Original).
• $100.00 registration fee (to be paid on day of registering child)
