HPV infection
The most common sexually transmitted disease (STD) in the USA
estimated 20 million currently infected and > 6 million new
infections per year. Approximately 12,710 women were diagnosed
with and 4,290 women died of cervical cancer in 2011.
Both women and men can acquire HPV infection by ge***al contact.
Most women clear their HPV infection within 1–2 years.
HPV is also associated with squamous cell carcinoma of the
anoge***al region and head and neck region.
LSIL is related to a transient HPV infection with little risk for
oncogenesis, whereas most HSILs are associated with HPV
persistence and hence signi fi cant potential for progression to
invasive cervical carcinoma.
HPV testing was introduced in the screening and management
guidelines for cervical carcinoma as a primary test with Pap test
cytology (co-testing for woman 30 years and oldest) or as a
reflex test in cases with atypical squamous lesion of undermined
significance (ASC-US).

case study?
This ThinPrep slide (low magnification) was examined,
and several areas were found to have “holes” that did not have
cellular material. When the slide was examined
for adequate cellularity using the method described
in The Bethesda System for Reporting Cervical Cytology 2nd
edition 2004 there was a mean of 40 cells per 10× magni fication.
What is the most appropriate evaluation
of this specimen?
(a) Unsatisfactory due to scant squamous cellularity.
(b) Satisfactory for evaluation. NILM.
(c) Unsatisfactory due to “holes” found in the cellu-
lar preparation.
(d) Satisfactory for evaluation. Epithelial cell abnor-
mality. LSIL. A more serious lesion cannot be
ruled out.

Liquid Based Cytology
Liquid-based cytology for cervical screening aims to improve the quality of the conventional cervical smear through an improved slide preparation technique.designed to produce a more representative sample of the specimen, with reduced obscuring background material
Cellular preservation is said to be enhanced, the preparation is more of a mono-layer and contamination (blood cells, pus and mucus) is reduced.
Currently 2 main systems are in use internationally :
