SCBA (Self Contained Breathing Apparatus) is a life saving unit in accidents (fire, release of toxic gases). But as it is a life saving device it must have a proper check and also must have proper training to cope up during emergency scenarios

Need a focus and attention to avoid such accidents

CONTROLS

Ensure that proper planning is conducted prior to lifts. This planning should include a review of drop zones, identification of safe zones while the lift(s) occur(s) and verification that all participants are trained in the work practices to conduct lifts and/or perform inspections;

Review the mechanical integrity program to verify that crane maintenance and inspections are being performed according to company, crane manufacturer and regulatory requirements. Special consideration should be given to:

accessibility and inspection of a crane’s boom hoist lock pawl cylinder assembly

outer diameter measurements of boom, main, auxiliary and pendant cables

documentation on the installation date, manufacture date and identification information for boom, main, auxiliary and pendant cables

inspection and lubrication of sheaves; and,

ensuring that wire rope size and sheave sizes are compatible;

Transformer explosion is generally due to an internal short-circuit in the transformer. The short-circuit, created by an insulation failure, reaches 1,200-degree Celsius. It vaporizes the oil located in the transformer tank. The oil, being in contact with very high temperature, vaporizes and creates explosive gases.

Controls
The only proven solution to prevent transformer explosion and fire. The TRANSFORMER PROTECTOR (TP), a passive mechanical system, is the sole proven solution to prevent transformer explosion. It is a Fast Depressurization System, as defined in Recommendations 850 of the National Fire Protection Association (NFPA).
Regular inspections and periodic maintenance help identify impending issues at the earliest and prevent future problems. Electrical transformers must be checked for damaged connectors, worn-out power cords, burning smell, loose plugs or misaligned parts to minimize system failures and fire hazards
