Joy crosses all boundaries when you come to know that one of your life dream has become a blatant truth in your life. From here forth you are no ordinary man, you have a prefix before your name I.e Dr. a dream that has turned into full blown bright day of summer. Here you are so “Congratulations”.

From here on you are entering into the next phase of life that seems so beautiful, exhilarating, one of service and above all of financial stability but with all this joy to accommodate, you have to make some space, you have to loose some of your inborn perks and privileges and one of them is your personal freedom.

After paddling through this exhilarating phase you might come across a new obstacle that is of Where and what to choose in house Job ?

First of all you all might know that it is the final year marks which determine your rank in the list of the House Job
For all the KMCITES
150 seats are reserved in KTH
50 in HMC
50 in LRH
(It varies with each session so before interview kindly confirm the number of seats)

House job is divided into two phases
-Junior House Job
-Senior House Job

You have to select one hospital for both phases. if you want to join KTH you should read ahead. For those who are opting for HMC and LRH they should consult with seniors who did their House Job there.

Khyber Teaching Hospital:
KTH, a hospital where you wandered since last three years has an aroma of being somewhere where you belong. Being affiliated with KMC, you might be knowing places and people as well as teachers and professors. As for as being a junior doctor most of the times you will be following orders. In General Wards ( Medicine and Surgery ) you would be doing the House Officer Job I.e phlebotomy, passing Catheters, NG and doing various other jobs as per directed by your seniors while in allied you might have first hand exposure of the patient include diagnosis and treatment as per your abilities, understanding and interest.

Interest is an important factor in learning during HJ. in HJ you are a free soul wandering here and there. No body expects of you, but if you are interested you might end up in doing procedures other can dream of. So the end point is, interest is very important.
We will start from Medicine

Medicine
HJ in medicine is compulsory that you can choose in either junior or senior house job as per your merits. Each medicine ward will take 9 HO initially that might be increased as needed. Each batch is then divided into two batches one consisting of 4 and other 5. Each medical units have one specific emergency day while in a month they have to do a special weekend i.e Saturday and Sunday and in the next week of special weekend a special Friday. In each medicine ward every house officer are allotted beds, 5-7 in number. Daily A house Officer’s Job is to take daily vitals, sometimes DPR in case Bed TMO is absent then follow all the orders and carries out any special orders as assigned by seniors. On cold day one House officer is on duty in day and one in night while on emergency day the whole batch will be on duty. Post emergency day is assigned to two HOs. The rota is mostly decided on mutual agreement.

There are four wards in medicine
Medical A:
As per House officer it is a hectic ward with high admission rate and with normal environment. Normal means you can leave after following orders and enviorment there is friendly (Do remember one man can turn a heaven into hell)

Medical B
One of the highest merit ward in our times, with optimal admission rate, friendly environment and less duties.

Medical C:
Same as B with extremely leniency as allowed per patient care. Highly recommended .

Medical D:
Highly toxic environment with attendance before 8 in the morning and not leaving till 1. Optimal admission rate.

My advice : Med C >Med B > Med A > Med D

friendly environment means that you can easily get leave by creating understanding with your collegues, less rate of explanations and less pre occupation with the presence of HOs .

You have to spend three months in the medicine ward of your choice.

Allieds:

You have to take medicine with its allied in either junior or senior HJ with sequence of your choice like whether you want to do first three months medicine and than allied or vice versa.

Medicine’s allied consist of
Pediatrics
Cardiology
Pulmonology
Skin
Psychiatry
Nephrology

Pediatrics:
One of the toughest ward in the house job. There are three units in KTH I.e A B C
It is one of the allied that has both morning and night duties.
6 HOs are allocated to each ward, 2 of which are performing their duties in Nursery while 4 will be in the ward. Their time distribution is like each house officer should spend two months in ward and one month in peads nursery.
HO job is similar to that of general medicine while in Nursery they have to present beds and do assessments. In ward you will have duty every third shift like you have a day duty today you next duty will tomorrow night.
Those who went in to these ward always complained of the hectic schedule while after passing out these hectic nights become a golden memory
As for as A , B and newly establish C is concerned environment is friendly in all ward, most of the people preferring “A”.

Cardiology:
4 house officers are assigned to cardiology unit. They are distributed in two pairs which are assigned in ICU and Cardiology ward as per their wish. morning round is compulsory with staying till 2. after 2 one house officers will remain there till 8, duty on every fourth day.( as per the tradition these days, sometimes they will oblige two house officer to stay till 8 then you will have to do duty on alternate days which become difficult.Our first two batches had this tradition, while the last two have duty every fourth day.
As per your interest it is one of the interesting ward where you can fulfill your childhood dream to see and give shocks, do CPR and other life saving procedures, understand ECG ( All based on your personal interest otherwise HJ is a job nobody will force you to learn any thing, learning is subjective in each & every ward). Environment is friendly.

Pulmonology:
In each rotation their are three HOs, with duty on every third day. Most of the time their patients are chronic and HO Job description is similar to that of Medicine with emphasis on ABGs. No night duty.

Skin & Psychiatry:
These are considered one of the most friendly and HO’s favourite wards where their job description is same as that of medicine with friendly environment, less number of duties and much leniency. You have enough time to learn meanwhile it is more observation based learning. Also there’s enough time to study for one’s exams, so highly recommended if one needs time for preparation of foreign exams. each is assigned with three House officer each. You will have to come daily for round in both wards. In derma one HO has to do 24hrs (which is generally till 10pm afterwards the HO is on call) so there’s a duty every 3rd day.
Psychiatry has 12hr duties, no nights. Even on 12hr duties there isn’t much to do so enough time for studying. Depending on your luck you can get weekends off. Easily the most chill and non toxic ward here.

Nephrology:
Most underrated ward with much less duties. Both morning and night shift are there. More emphasis on skills like passing CVP and double line. Male Drs in ward are assigned to do nights while female are assigned to days.based on that your duties/month depends upon the no of house officers. they mostly take in 6 HO.

In case you do your first House Job in medicine than there will be another interview in which you will choose Surgery or vice versa

Surgery:
Surgery HJ is considered less hectic and more friendly as compared to that of medicine. surgery seems very cool when you come out of the hectic schedule of medicine and Allied. Like General Medicine each General surgery ward is assigned 9 HOs in each rotation with similar duties pattern. Batch division is the same while special emergency differs in a way that Friday is divided between surgical A and B alternatively while Saturday between C and D and each ward do one Special Sunday in one month. Like Medicine, In some wards on out day two a House officer are assigned on OT day. There is no round on OT day so those House officers which are not on duty can have their leave on that day (although officially not allowed).

Meanwhile ward distribution are as follows

Surgery A:
Heaven of House officers. Optimum Friendly environment, with less patient burden.

Surgery B:
Second in this category but with more restrictions like early morning 8AM presence, compulsion of wearing overall in round, friendly environment, over documentation but with extremely lenient duties and with lesser patient burden than any other surgical ward.Moreover you are not obliged to take blood with quite facilitative paramedic staff.

Surgical C:
With Friendly environment it is the one of the highest patient burden ward in province. All time hectic ward with extremely tiresome job is that of house officer. If you are an adventurer and want to do any sort of life time adventure this is the place for you as house officers. Kindly note, if you are passionate about surgery you must have time to observe it which is among the impossibilities in this ward. if you are in your senses and want optimal job burden that you can carry then stay away, this is not the place. Don’t go after big names they are not going to do your job for you.

Surgical D :
Most underrated ward in Surgery, with more patient burden than A and B but with leniency and friendly environment.

My Advice: Sur A > Surgical B >Surgical D > Sur C

Orthopaedic :
Most relax wards in all the allied with three units. Each unit takes two house officer which perform 24 hours duty on emergency day. There are two emergency in one week so one house officer have one duty/week. Good for study.

Eye:
Two units, seats vary across the rotation but officially three in A and 4 in B units . Relax as per info.
Duties distribution depends on HO. On emergency day everybody comes daily for OPD while one HO does 24hr duty. On cold days depending on the ward you might have to come for attendance or not at all. No cold duties. Due to the independent duty at night one can learn a lot depending upon one’s interest.


ENT
Two units with three and Four House Officer , 4 in A and 3 in B. emergency duties is divided between 2 HOs (12hrly) . ENT A is reported to be much relaxed than its other counterpart .
First half of the batch have the privilege to have TMO on their duty rota so duties per month are much lesser than the second half. (First half means first three months of 6 month rotation)

Plastic Surgery:
Three house officers with every third day 24 hours .

Gynae and Obstetrics:
Three units each with most laborious ward in surgical allied . Duty is much variable in each unit. Kindly manage to ask your seniors about schedule in this ward .

Peads surgery:
Four House officers with 24 hour on every 4 day.

Track to Sharan, one of the densest forest of Pakistan starts from Paras, a small town of between kaghan and Naran, from where unpaved Jeep track overlooking kunhar river will take you to the most mesmerizing meadows exactly opposite to sirriyan payan meadows.

One of the most humble personality that we had in our time , Mr. Abdul Basit is no more with us.
May Allah bless him with high ranks in Jannah and give forbearance to the grieved one on this irreparable lost.

Walking through these roads took me back to the time, when stress and frustration was in the air.when Weather outside won't bother us

Chukail Meadows & Gabeen Jaba a post KMC visit was indeed a sole opportunity to for getting togather and revisiting our roots

At last it was the time which costs all!
Reunion

The bump road, calling it a road would be a misnomer though, we reached to the last stop of Hercules Humvee of locals.We started hiking right from the dense pine forest through the crests and trough of river gorge through the flooded round stone followed by a steep ascend into unending meadows of Chukail.Chukail banda, being deserted in this part of year looked like an abandoned monastery guarded by the steep snow covered peaks of swat.This one days ascend and decend does make us weary which was slightly compensated by the long walk on the bank of river swat in behrain.
Next day, Another amazing scene was waiting for us across the dangerous turns of Gabeen Gaba, where unending meadows were looking our way.
