INSTRUCTIONS
a. Form will be filled by the candidate, with his own handwriting. Typing is not acceptable.
b. Following documents must be attached with admission form:-
(1) Five latest passport size photographs duly attested on front side by the Headmaster/Principal of the school, pasted on the form / call letter slips.
(2) Four envelopes address must be written in bold letters on all envelops.
(3) Bank draft number ___________ dated __________ for Rs. 1160/- (in original) as Registration Fee / Postage Charges (NOT REFUNDABLE) in favour of Commandant Military College Jhelum payable at Habib Bank Limited Sarai Alamgir (Code 0658). Bank Draft other than of Habib Bank will not be acceptable. (CASH WILL NOT BE ACCEPTED).
(4) Documents Mandatory for Confirmation of Date of Birth
(a) For Army Officer (Serving / Retired). Photocopy of CORO Part-II Order regarding date of birth of the candidate, Form-B and Father’s NIC will be attached.
(b) For Civilian. Photocopy of computerized Form-B of the candidate and Father’s NIC will be attached.
c. Deadlines for Submission of Application Form
(1) Last date for submission of admission form to Military College Jhelum is 30 September 2013.
(2) Late fee of Rs. 500/- will be charged for applications received between 1-10 October 2013 and Rs. 1000/- from 11 to 20 October 2013.
(3) After 21 Oct 2013 a fine of Rs. 1500/- will be charged and the application form will only be accepted by hand at Military College Jhelum. The candidate, in this case, will also appear for his written test at Military College Jhelum.
