Performed last week.
34 Y F, with huge left renal vein and tumor thrombus extending to, and involving the IVC wall anteriorly, with enlarged para-aortic nodes and no distant mets. Left radical nephrectomy, para-aortic nodal dissection and resection of about 2cm of IVC anterior wall performed. Angioinvasion of a small portion of the IVC just above the right renal vein was also noted, which was excised separately. The supra-renal IVC was exposed after dissecting the right liver ligaments and rotating it to the left and the lumber veins were dissected and cut to Mobilize the infrarenal IVC for a primary repair . A difficult one due to the IvC wall invasion. Thanks to Dr Azam Shoaib (HPB surgeon) and Dr Qurrat’ our anesthetist and our fellows Dr Sulaiman and Dr Gulzar and the resident Dr Abbas for the support throughout.
It was a 14 hours long operating day, where performed this case after a Pancreaticoduodenectomy. Thanks God, both patient recovered well and discharged home.
