مهرباني ڪري فارم ڀرڻ لاءِ هيٺ ڏنل ضروري دستاويز گڏ آڻيا وڃن:

سڃاڻپ ڪارڊ (NIC)
رابطو نمبر (Contact Number)
سڀ شاگردياڻيون مقرر وقت تي اسڪول ۾ اچي پنهنجا فارم مڪمل ڪن
