Color vision test or Ishihara Test (negative result for color blindness) from any DOH-accredited hospital or clinic.
Recent 2x2 picture taken within 3 months

TRANSFEREES
Original copy of the Transcript of Records (TOR) from the previous school, which must include the descriptive title of the subjects taken, the number of units and the grades earned.
Certificate of Transfer Credentials (Honorable Dismissal) from the school last attended.
Certificate of Good Moral Character coming from School Head of the previous school.
PSA Birth Certificate (original copy)
Color vision test or Ishihara Test (negative result for color blindness) from any DOH-accredited hospital or clinic.
Recent 2x2 picture taken within 3 months
