✅️Photocopy of PSA Birth Certificate

Procedure
Step 1: Secure a copy of enrollment form and agreement form at the Guard post.
Step 2: Proceed to Kinder Sullano to read and accomplish all the forms.
Step 3: Proceed to the library for Literacy and Numeracy Assessment for Kinder and Grade 1.
Step 4: Proceed to Kinder Herrera for Validation and Submission of Enrollment Form.

☑️Original SF9 (report Card)
☑️Photocopy of birth certificate
The assigned personnel will check and receive your documents.

Reminders:
-Parent/Guardian must accompany the students during enrollment
