• Academic Records (Report Card or Form 137)
• College Admission Test (CAT) Result (Printed Copy)
• PWD Card (required for Persons with Disabilities applicants)
• Certificate of Indigency (issued by the Barangay)
• OWWA Certification (required especially for applicants with OFW parents)
• NCIP Card (required for Indigenous Peoples applicants)

Please make sure to keep a personal copy of all documents for your reference.
