Kindly fill out the TSU-VPA-SF-39 form and attach your 2x2 picture.

After accomplishing the form, please go to the office of the College Dean and get your assessment at the College Clerk. Please then settle your balance for the assessment, amounting to PHP 800 at the University's Cashier.
