For the information of everybody, due to COVID-19 pandemic, the enrollment procedure is modified taking into consideration the safety of the students, parents and teachers of Tandag National Science High School.

Please take note of the following:

1. The first Two weeks of enrollment (June 1-12, 2020) shall be done remotely by (Either of the following ways):

a. Accomplishing the Online Learner and Survey Form (LESF) https://bit.ly/3cjqKEM , posted in the page of the school, or;
b. Submit filled up downloadable form thru email, messenger or any means of electronic submission of the school to the adviser of the previous school year. Absolutely no face to face. Or;
c. By Phone Call (landline, mobile phones) to the adviser (previous adviser, S.Y. 2019-2020).

2. Physical Enrollment:

Physical submission of Learner and Survey Form (LESF) will start on the Third week of enrolment period (June 15-30, 2020) in case the parents failed to enroll their child online. Parents/Guardians may accomplish the Learner Enrollmet and Survey Form (LESF) and submit to any of the following sites:

a. School Enrollment Booth (TNSHS)
b. Red Cross Training Hall
c. Bag-ong Lungsod Barangay Hall
d. Bongtod Baranay Hall
e. Dagocdoc Barangay Hall
f. Telaje Barangay Hall

3. For TRANFEREES:
Learners planning to transfer from one school (“the originating school”) to another (“the receiving school-TNSHS”), the originating school shall directly contact the receiving school through the landline or school Hotline numbers.

4. For BALIK-ARAL:

All Balik-aral enrollees to be registered shall directly contact TNSHS thru the school’s contact numbers.

5. For GRADE 7 STUDENTS:

NO ENROLLMENT yet, the school is still waiting for the guideline from the DepEd regional office. Information regarding Enrollment for grade 7 will be announced as soon as the guideline is available for implementation.

To All Parents/Guardians:

In preparation for the opening of classes on August 24, 2020, we humbly request you to fill in this online LEARNER ENROLLMENT AND SURVEY FORM for S.Y. 2020-2021 on or before June 30, 2020 at 5:00 PM.

This is done to collect basic profile of the student as registrant and gather information on household capacity to access various modalities (i.e available gadgets, possible facilitators, etc.) in the New Normal School scenario for the school year 2020-2021.

Your responses will be dealt with utmost confidentiality.
