📢 NOTICE TO ALL OJT STUDENTS

Please be informed of the following important deadlines and requirements:

1. DTR SUBMISSION

Deadline: December 1–9, 2025

Make sure your Daily Time Record (DTR) is complete and properly signed before submission.

2. STUDENT EVALUATION FORM

Deadline: December 1–12, 2025

Please submit your Student Evaluation in a sealed white envelope.

Unsealed or incorrect envelopes will not be accepted.

3. OJT MONTHLY MEETING

Venue: CHMSU Gymnasium

Tentative Date: December 9, 2025

Please wait for further confirmation and be present during the meeting.

4. FOR STUDENTS WHO HAVE COMPLETED 600 HOURS

You may now submit your Clearance and Certificate of Completion for 1st Semester processing.

⚠️ Important Reminders:

Those who have not yet started their deployment are not required to submit for the meantime.

Each group/company may designate one (1) representative to submit the hard copies at the OJT Office (depending on your group’s arrangement).

Ensure that the Internship Agreement is officially signed on all pages by the required signatories, especially on the last page.

Provide one (1) valid ID copy of each signatory together with three (3) specimen signatures for documentation and notarial purposes.

Failure to submit on the specified date will mean that the OJT Office will no longer accommodate the notarial expenses.

OJT Orientation with Parents | CHSMU Gymnasium – August 8, 2025

A day of guidance, preparation, and shared commitment to excellence. The program began with warm welcome remarks from Dr. Angelie P. Lopez, Dean of the College of Industrial Technology, followed by an insightful discussion on OJT guidelines for AY 2025–2026 led by Dr. Joven E. Tuburan, BSIT OJT Supervisor, with the support of Sharjolyn Mae Zaragoza, OJT Office Staff, together with students and staff. Hosted by former BSIT Governor Rafael Baylosis, the event concluded on a high note, filled with energy and purpose.

This gathering aimed to equip parents and students with knowledge of their responsibilities, the CMO 104 Series of 2017 guidelines, and the OJT requirements for the upcoming deployment to various industries. Represented were different majors from Cluster A — Automotive, Mechanical, Refrigeration and Air-Conditioning, Electronics, and Electrical — and Cluster B — Architectural Drafting, Food Trades, and Fashion & Apparel.

With nearly 700 attendees, it was more than an orientation — it was a united step toward the brighter future and growth of BSIT CHSMUans.

Personal Data Sheet (PDS)
• Attach a 2x2 ID picture

OJT Application Form
• Must be signed by your parents

Pledge of Conduct
• Must be signed by your parents

Parental Consent Form (5 original copies)
• 1 for the OJT Office
• 1 personal copy
• 1 for the Host Training Establishment (HTE)
• 2 extra for other official uses
• Must be notarized

Note: If you're not yet sure about your assigned company, you may leave the company section blank for now.

Internship Agreement (5 original copies)
• 1 for the OJT Office
• 1 personal copy
• 1 for HTE
• Others as needed
• To be submitted once you are deployed
• Must be signed by the assigned mentor

Enrollment Form (EF)
• Must show the enrolled OJT subject for 1st Semester, AY 2025–2026

Paid Student Insurance
• 1 copy from the Office of Student Affairs (OSA)

Copy of Prospectus with Grades
• From 1st Year to 3rd Year

Barangay / Police / NBI Clearance (Optional for OJT Office)
• Required only if the company asks for it

Physical Examination Results (Required if deployed outside the Negros Region)
Includes:
• Vision Test
• Chest X-ray
• Drug Test
• Other lab results

Medical Certificate (For deployment within Negros)
• Must be issued by a DOH-accredited public clinic or hospital
• You may request a referral letter from the OJT Office

Note:
Please compile all documents neatly in a Long Green Folder.
Ensure that your documents are complete and well-organized before submission.
For students who are unsure of their preferred Host Training Establishment (HTE), you may leave the HTE section blank for now.
However, if you are already settled on your preferred HTE for deployment, you may start submitting your requirements at the OJT Office begins on August 11, 2025.
