Deadline: December 1β9, 2025
Make sure your Daily Time Record (DTR) is complete and properly signed before submission.
2. STUDENT EVALUATION FORM
Deadline: December 1β12, 2025
Please submit your Student Evaluation in a sealed white envelope.
Unsealed or incorrect envelopes will not be accepted.
3. OJT MONTHLY MEETING

Those who have not yet started their deployment are not required to submit for the meantime.
Each group/company may designate one (1) representative to submit the hard copies at the OJT Office (depending on your groupβs arrangement).
Ensure that the Internship Agreement is officially signed on all pages by the required signatories, especially on the last page.
Provide one (1) valid ID copy of each signatory together with three (3) specimen signatures for documentation and notarial purposes.
Failure to submit on the specified date will mean that the OJT Office will no longer accommodate the notarial expenses.

OJT Orientation with Parents | CHSMU Gymnasium β August 8, 2025
A day of guidance, preparation, and shared commitment to excellence. The program began with warm welcome remarks from Dr. Angelie P. Lopez, Dean of the College of Industrial Technology, followed by an insightful discussion on OJT guidelines for AY 2025β2026 led by Dr. Joven E. Tuburan, BSIT OJT Supervisor, with the support of Sharjolyn Mae Zaragoza, OJT Office Staff, together with students and staff. Hosted by former BSIT Governor Rafael Baylosis, the event concluded on a high note, filled with energy and purpose.
This gathering aimed to equip parents and students with knowledge of their responsibilities, the CMO 104 Series of 2017 guidelines, and the OJT requirements for the upcoming deployment to various industries. Represented were different majors from Cluster A β Automotive, Mechanical, Refrigeration and Air-Conditioning, Electronics, and Electrical β and Cluster B β Architectural Drafting, Food Trades, and Fashion & Apparel.
With nearly 700 attendees, it was more than an orientation β it was a united step toward the brighter future and growth of BSIT CHSMUans.

Note:
Please compile all documents neatly in a Long Green Folder.
Ensure that your documents are complete and well-organized before submission.
For students who are unsure of their preferred Host Training Establishment (HTE), you may leave the HTE section blank for now.
However, if you are already settled on your preferred HTE for deployment, you may start submitting your requirements at the OJT Office begins on August 11, 2025.
