To ensure proper coordination and effective medical coverage during activities and events, SERT respectfully reminds all organizations, departments, and event organizers of the following requirements:

OSA Approval Required
SERT will only accept request letters that have been duly endorsed and approved by the Office of Student Affairs (OSA). Please secure the necessary approval before submitting your request.

Submit Two (2) Copies of the Request Letter
Organizations are required to submit two (2) copies of their approved request letter:
โข One (1) copy shall be submitted to ๐๐๐ง ๐ฟ๐๐๐ฉ๐๐๐ง ๐พ. ๐๐๐ฃ๐๐ฉ๐ค, ๐๐พ๐ง๐๐ข through the PPMGS Office.
โข One (1) copy shall be submitted directly to the SERT Office for documentation and coordination purposes.
Submitting both copies ensures proper endorsement, record-keeping, and timely coordination between SERT and the concerned offices.

Submit Requests Ahead of Time
Organizations are strongly encouraged to submit their request letters at least one (1) week before the scheduled event.
Please be advised that requests submitted less than three (3) days before the activity may result in limited manpower availability. SERT members are not only responders but also students with academic responsibilities, examinations, classes, and prior commitments. Early submission allows us to properly coordinate schedules and deploy sufficient personnel to ensure the safety of your participants.
For this reason, SERT cannot guarantee a full-force deployment for late requests.

Provide Ice for the Ice Box
To help maintain the readiness of our medical supplies and emergency equipment, organizers are requested to provide ice for the SERT ice box throughout the duration of the event.

Provide a Designated Medic Station
Please allocate a safe and accessible area where SERT personnel can establish a medic station, store equipment, monitor participants, and respond efficiently to emergencies.
