Releasing of Batch 2014 Yearbooks feat. KATUPARAN starts today Apr. 02, 2019.

Don't forget your document and kindly bring eco-bag.

KATUPARAN: 2014 Yearbook Release

Due to the pro-long reconciliation of the Masterlist for the Paid Yearbooks from the record of Accounting office, scheduled release will start on APRIL 02, 2019 9AM~6PM only at Learning Resource Center of the University.

It will be available daily except Weekend and Public Holidays.

Please be reminded of the following:

1. If you are currently employed, you need to submit the following:
a. Whether Original Receipt is Available or Not, you STILL need to submit:
1-Page Document which includes photocopy of your Company ID (back-to-back if applicable) and one (1) Valid ID (Government IDs, Passport, PRC, etc).

2. If not employed at the moment, you need to submit the following:
a. Original Receipt (O.R.) of Paid Yearbook (if still available).
b. If NOT, photocopy of two (2) any recognized/valid IDs.

3. If not able to get your Yearbook, Authorization Letter is needed together with:
a. Photocopy of ID for the owner of the Yearbook AND
b. Photocopy of ID for the authorized person

4. In the case that your name NOT Included on the Masterlist of the Paid Yearbooks from the record of Accounting, you will need to provide the following:
a. Original Receipt O.R. of photocopy of O.R. indicating your name and/or amount paid
OR
b. Intent Letter indicating that you paid the Yearbook with at least one (1) person witness of your payment.
OR
c. Affidavit of loss (in case you have lost your O.R.)

5. Kindly surrender Alumni ID or any valid ID from guard house upon entering the University.

6. Please wear decent attire upon entering the University to avoid any inconvenience.
