Submission of requirements and the schedule of medical and dental examination will be accommodated until further notice. 𝗖𝗹𝗮𝘀𝘀𝗲𝘀 𝗼𝗳𝗳𝗶𝗰𝗶𝗮𝗹𝗹𝘆 𝗯𝗲𝗴𝗶𝗻 𝗼𝗻 𝗝𝘂𝗹𝘆 𝟲, 𝟮𝟬𝟮𝟲.

Please be reminded of the following program information:
📌𝘐𝘯𝘤𝘰𝘮𝘪𝘯𝘨 𝘎𝘳𝘢𝘥𝘦 11 𝘴𝘵𝘶𝘥𝘦𝘯𝘵𝘴 will follow the Strengthened SHS Curriculum under the Trimestral Modality
📌𝘐𝘯𝘤𝘰𝘮𝘪𝘯𝘨 𝘎𝘳𝘢𝘥𝘦 12 𝘴𝘵𝘶𝘥𝘦𝘯𝘵𝘴 will continue under the existing curriculum with 2 semesters

✔ Screenshot/Proof of completed Pre-Enrollment Google Form
✔ PSA Birth Certificate (Original)
✔ Report Card (Form 138) for JHS Completers
✔ Learner’s Permanent Record (AF-5) for ALS Graduates
✔ Good Moral Certificate
✔ ESC SHS Voucher Certificate (for private school students)
✔ QVR Certificate (for QVR Grantees)
✔ Blood Type Result from any DOH-accredited diagnostic laboratory
✔ 3 pcs 2×2 ID picture (white background)
✔ Long Brown Envelope for all documents

Additional Requirements for Tech-Voc Maritime (Grade 11 & 12):
✔ Ishihara Test Result
✔ Hearing Test Result
