Attention to all officially registered 1st year and 2nd year students for A.Y. 2021-2022, you may now send your request for issuance of school ID through the following steps;

1. Prepare a soft copy of your recent photo
2. Prepare a soft copy of your signature
3. Login to your university email and fill-up the online Student ID Request Form found here; bit.ly/bisumcid
4. Wait for the confirmation of your submission

Note: Please observe the following guidelines for the student photo and signatures to be uploaded. Submissions that does not follow these guidelines will be returned, and will be required to be resubmitted.

Student Photo Guidelines
✅ Format must be .jpg and file size of no more than 10MB
✅ Background must be WHITE and with NO SHADOWS
✅ Wear WHITE COLLARED SHIRT
✅ Eyes on the camera, with minimal smile, and with NO SHADOWS on the face
✅ Follow the pose in the sample photo, and fix your hair
✅ Don't wear hat/head covers, and strictly no earrings for males

Student Signature Guidelines
✅ Format must be .jpg and file size of no more than 5MB
✅ Background must be WHITE and with NO SHADOWS
✅ Signature must have CLEAR and THICK lines

We will be posting schedules for pick-up of your student ID on this page, once they're finish printing. (Note: you will have to personally visit the campus to pick-up your student ID.)
