QUALIFICATIONS:
· Must be an INCOMING 2ND YEAR COLLEGE STUDENT FOR THIS COMING AY 2025 – 2026.
· Must be under the STEM Courses, Psychology, Social Work, and Education.
· Must have a GWA of 2.25 85% or better with NO FAILING GRADES and REGULAR STUDENTS
· MUST NOT BE A RECIPIENT OF OTHER SCHOLARSHIP PROGRAM (PARTIAL/FULL)
· Must be DEDICATED to attend Charity First Activities every 1st Sunday of the Month
· Metro Manila Residents ONLY not residing in dormitory.
REQUIREMENTS:
(Kindly provide physical copies and submit them to the Office of Scholarship and Financial Assistance, located at W119 beside the Sampaguita Building.)
· Accomplished Charity First Application Form
· Complete Grades (SHS and 1st Year 1st Sem Grades)
· Latest University / School Registration Form
· Autobiography or Talambuhay
· Latest Copy of Utility Bills and Printed Family and House Picture (inside and out)
· PSA Birth Certificate, School and Government ID’s 2x2 ID Picture and Certificate of Indigency
· Social Case Study Report get it in your nearest City Social Welfare Development Offices
· DSWD 4p’s ID of you or any family members (if applicable)
· Latest ITR OR Parents’ payslip (if applicable)
· Sketch of Home Address from Major Landmark for Home Visitation purposes
· Copy of Medical and X-Ray Result

· ALL REQUIRED DOCUMENTS MUST BE SUBMITTED AS PHYSICAL COPIES TO OSFA, W119, PUP MABINI CAMPUS. PLEASE LOOK FOR MS. ROCHELLE OR MR. JASON.
· Shortlisted applicants will be notified and will undergo a document assessment or a face-to-face interview at the Office of Scholarship and Financial Assistance, located in W119 or Charity First Foundation, Inc. Office.
