This is to formally announce the signing off of Ms. Mariniel E. Ando from PIT Tabango Registrars Office effective Monday 4th of September, 2023.

As such, this has caused necessary adjustments in the offices' work load. This Office is very appreciative of your patience with the delays in the delivery of services that may come as a result.

(SHS graduates / HS graduates)

•Original Form 138 (SHS/HS card)
•Original Certificate of Good Moral Character
•One (1) piece recent 2 x 2 ID picture, colored in white background with nametag
•Photocopy of PSA/NSO issued Certificate of Live Birth
•Photocopy of PSA/NSO issued Certificate of Marriage ( for married women )

ALS Passers

•Original Certificate of Rating
•Original Certificate of Accreditation and Equivalency (A&E) Test
•Original Certificate of Good Moral
•Photocopy of PSA/NSO issued Certificate of Live Birth
•Photocopy of PSA/NSO issued Certificate of Marriage ( for married women )

Transferees

•Original Informative Copy of Transcript of Records
•Original Certificate of Honorable Dismissal
•Original Certificate of Good Moral Character
•One (1) piece recent 2 x 2 ID picture, colored in white background with nametag
•Photocopy of PSA/NSO issued Certificate of Live Birth
•Photocopy of PSA/NSO issued Certificate of Marriage ( for married women )
